Abstract

Prevention of microangiopathy of retinal vessels through tight compensation of diabetes and the risk factors form the basis of successful treatment of diabetic retinopathy and maculopathy. At present, we are able to succesfully treat developed diabetic retinopathy as well as maculopathy, including stabilisation of visual function, subject to early diagnosis of retinal changes.

In collaboration with a diabetologist, fenofibrate s are used as the first line treatment hen the inner blood-retinal barrier is disturbed (dammaged tight junctionsbetween endothelial cells of retinal vessels). To reduce focal macular edema , the sources of fluid leakage into central retina are occluded using laser interventions on microaneurysms.

Laser therapy is the fold standard in the treatment of diabetic macular edema as well as diabetic retinopathy. Some authors prefer surgery technique (pars plana vitrectomy with internal lifting membrane peeling) even at early stages of diabetic retinopathy and maculopathy.
