Clopidogrel and proton pump inhibitors

Abstract

The question of concurrent administration of proton pump inhibitors and clopidogrel is increasingly discussed by the professional community. Co-administration of these drugs occurs in clinical practice quite often.

Several retrospective and observational studies suggest a potential interaction between clopidogrel and PPI, which would lead to a reduction of the antiplatelet activity of clopidogrel and therefore to an increasing of the incidence of thrombotic events. Overall, we can not reliably assess whether the observed relations are a manifestation of adverse interactions, indirect connections, or just a random phenomenon.

From the only currently available randomized prospective study, which has been prematurely terminated, was not confirmed a higher incidence of cardiovascular events or subsequent revascularisation.
