We describe an extra-anatomical bypass from the right common carotid artery to a patent ilio-femoral crossover bypass in a high risk patient with an occluded aorta, bilateral subclavian artery disease, and a critical ischaemic left leg. An occluded right internal carotid artery allowed us to use the right common carotid artery as a donor vessel for carotico-iliac bypass without risking steal syndrome.
