Abstract
The article comes up with the assumptions that have been used as a fundament for generating of the screening evaluation scheme for risk senior patiens (aged over 65). This is actually the second part of the study called 'Senior patients' discharge process from the acute beds in the hospital' that has been conducted by the Institute of medical ethics and nursing - 3 Medical Faculty of Charles University Prague.
The main objective of the study is to recommend the most convenient procedure for senior patients' discharge from the hospitals as well as to design the competence of each member of the multi-disciplinary team. The second stage of the study was focused on appropriate tool's design that would be based on the structure of complete geriatric assessment (CGA) and, in the same time, would cover the key areas of the functional conditions and, moreover , to test its using under the existing conditions in the hospitals.
