Sine Syndromes in Clinical Manifestation of Dermatomyositis

Abstract

This paper deals with the problem of sine syndromes in clinical manifestations of dermatomyositis. It presents an overview of the literature of sine syndrome, which is a disease that does not fulfill the diagnostic criteria of dermatomyositis but can have a severe course.

In the case report, we describe the development of acute interstitial pneumonia in the course of systemic polymyositis without clinical signs of muscle involvement, but with positivity of anti-Jo 1 antibodies and elevation of myoglobin and troponin. Electromyography did not confirm myositis or a disorder of neuromuscular transmission.

The case report points to the need for rheumatologic and immunological examination as well as pneumologic examination of pulmonary interstitial involvement within sine syndrome. It also shows the success of corticosteroid treatment.
