Deficits in information transfer between hospital-based and primary-care physicians, the case of kidney disease: a cross-sectional study

Abstract

Kidney dysfunction, acute or chronic, does frequently occur in hospitalized patients however is frequently not noticed or included in the diagnostic summary of the medical report for primary-care physician which can have a negative effect on subsequent care for chronic kidney disease patients.
