Lowering the risks of organ complications is the ultimate target in the treatment of diabetes mellitus. Due to the progressive character of diabetes mellitus it is difficult to maintain the target metabolic compensation as a tool for preventing complications; therefore many patients are inadequately compensated for years.
The intensified treatment is initiated too late, when organ complications are often already manifested. The following text is focused on obstacles to the initiation of the insulin therapy, and the improvement of patients' adherence that leads to both patients' and physicians' satisfaction.
