The article presents an update of crucial recommendations defined by GOLD initiative (Global Initiative for Cronic Obstructive Lung Disease, GOLD) in 2017 Global Strategy for the Diagnosis, Management, and Prevention of Chronic Obstruktive Pulmonary Disease, COPD. The guidelines pay special attention to the symptoms and exacerbations of tha disease, deal with certain pharmacological and non- pharmacological approaches, and even change the definition of COPD.
From the purely pharmacological point of view, the severity of obstruction becomes less important. At the same time, GOLD promotes not only escalation, but also de-escalation of therapy.
The treatment shifts towards a much more pesonalized one with respect to both COPD and comorbidities.
