The treatment must be complex, containing offloading of the affected foot, revascularization procedures in presence of ischemia, antibiotic treatment if infection is present, debridement and proper wound bed preparation and optimal wound care choice.
It seems that the most important role in healing process plays functional microcirculation. In support of microcirculation we can use non-pharmacological and pharmacological methods as alprostadile or sulodexide.
