Patients with autoimmune inflammatory rheumatic disease (AIIRD) are at high risk of infec- tions as a result of multiple factors. All patients with AIIRD should be vaccinated against pneumococcus and influenza prior commencing an immunosuppressive therapy, vaccination against hepatitis B should be considered in at risk individuals.
Vaccination can be performed also during ongoing therapy (except B-lymphocyte depleting therapy) but is preferred during a stable disease with low disease activity. Some treatment modalities may alter the immuno- logic response, but this is usually satisfactory to provide seroprotection.
Vaccination with live attenuated vaccines is not recommended in patients on immunosuppressive therapy, but may be considered undercertain circumstances, such as prior commencingtherapy with inhibitors of Janus kinases (JAK).
