The article describes a case report of a 22-year-old patient who competes in road cycling. He has been treated for type 1 diabetes for 10 years and is free of chronic diabetic complications.
From the first year of diabetes he was treated with an insulin pump. However, diabetes compensation has long been unsatisfactory.
After the transition to our department, he underwent a comprehensive re-education, but the situation did not improve significantly even when we finally motivated the patient to cooperate. The patient did not manage to adjust insulin doses during pump treatment.
Finally, the solution turned out to be a multiple injection regimen with insulin degludec as basal insulin and insulin faster aspart as prandial / correction insulin. The patient proceeds according to the prepared dosing table and in his case it has proved to be advantageous that he can no longer intervene in the insulin dose as much as with the insulin pump treatment.
The problem of nocturnal hypoglycaemia following training sessions is also solved with this regimen with insulin degludec as a basal insulin. The patient's diabetes control has improved significantly, he feels well, and his sport's results have started to improve as well.
