Spasticity in multiple sclerosis

Abstract

Spasticity in multiple sclerosis is a relatively common sign. Upper motor neuron syndrome consists of muscle overactivity, shortening and paresis.

The main goal of spasticity treatment is to decrease the negative effect of spasticity on daily activities limiting the patient. We do not try to eliminate spasticity completely because increased muscle tone can help, for example, when standing or walking.

Pharmacotherapy is the most common way of spasticity treatment. Baclofen and tizanidine are mainly used, often in combination with each other.

In severe generalized spasticity, a very good clinical effect of intrathecal baclofen administration through pump delivery systems has been demonstrated. It can be combined with botulinum toxin administration in focal muscles.

Rehabilitation has in important role; it is necessary to start as soon as possible, usually at the beginning of the disease.
